U-Card Submission Form

Please submit the following information to activate your U-card for NMR access.
U-cards will be activated on a weekly basis, usually on Thursdays.

Please send this information only ONCE. If your card does not work, check with Letitia Yao in the NMR facility.
If you are a newly trained user of the mass spec or X-ray facility and already have NMR access,
email Letitia to add your card number to those doors. Do NOT resubmit your card number.
